About our PCN
The Coast Mountain Primary Care Network (PCN) is a collaborative partnership between local First Nations, the Nisga’a Valley Health Authority, Northern Health, the Pacific Northwest Division of Family Practice, and the Ministry of Health. The PCN serves a large and diverse region, including Terrace, Kitsumkalum, Kitselas, Nisga’a Valley, Tahltan territory, Dease Lake, Stewart, and surrounding areas.
Indigenous-led community PCN Clinics are an important part of the PCN. These clinics are guided by the communities they serve and are designed to provide culturally safe, community-based primary care. By building on existing resources, partnerships, and local knowledge, these clinics help improve access to care for individuals with diverse and complex needs.
The Low Barrier Clinic and Mobile Outreach Team helps improve access to care by meeting patients where they are. These services focus on providing culturally safe, relationship-based care, particularly for individuals who may face barriers accessing the healthcare system. The team works alongside patients to build trust, support wellness, and strengthen connections to care, community, and culture.
The Maternity Clinic is a collaboration between Northern Health and the Coast Mountain PCN, providing wraparound prenatal and postnatal care for expectant mothers, babies, and families in a culturally safe, community-based setting in Terrace.
